Progressive Ataxia and Palatal Tremor Syndrome with Monocular Pendular Nystagmus

Abstract

|
Progressive ataxia and palatal tremor syndrome (PAPT) refers to an idiopathic disorder characterized by rhythmic oscillation of the palate in addition to progressive cerebellar symptoms. Various eye movement abnormalities may occur in PAPT. A 61-year-old man presented with dysarthria, ataxia and imbalance for 3 years. Examination additionally revealed monocular torsional pendular nystagmus in the right eye and symmetrical palatal tremor. MRI disclosed cerebellar atrophy and bilateral hypertrophic degeneration of the inferior olivary nucleus without a focal lesion involving the Gullain-Mollaret triangle. Monocular pendular nystagmus may occur in PAPT. PAPT should be considered in patients with oculopalatal tremor without structural lesions.
